Transfer purified fat to breasts and face

Lipotransfer or fat transfer to breasts is now becoming a popular request in my practice especially among young women with small breasts who do not desire surgery for breast augmentation and older women with post pregnancy loss of volume causing deflated looking breasts and sagging breasts. There is also an increase need for filling of the face to recreate a 3D effect which fats can do better then synthetic fillers. 

Lipotransfer or Fat Grafting has been conducted worldwide since many years ago but the processing of the harvested fat was very tedious and time consuming. And if the harvested fat is not purified properly or adequately, the implanted fat will degenerate very fast making its effect last only 9-12monrhs, similar to implanted  hyaluronic acid fillers. With better purification of extracted fat, the implanted fat lasts longer ie upto 5 years and has a higher survival rate in recipient site. 

In Nov 2015, I was in Tokyo Japan for a short course on Fat Grafting and Lipotransfer using the Puregraft. Puregraft is a device that enables the purification of harvested fat without the need for centrifugation which is traditionally  done. It uses a special filtration system that enables the fat graft to go through but traps any debris, fibre and unwanted agents within the graft. 

The training was conducted by Prof Yoshiro Kamakura at his classy Shishin Aesthetic Surgery Clinic right in the heart of Roponggi Tokyo. Roponggi was so happening at this time of year as it was Halloween weekend.

It started with a brief lecture on principals of fat grafting and transfer to breasts as well as the advantages of using Puregraft to purify the extracted fat before transferring. This is as opposed to the traditional method of spinning the fat to separate it from the oil and anesthetic fluid as well as debris.

Puregraft enables the fat to be cleansed and purified without centrifugation and can be transferred a lot faster then traditionally. The purified fat allows for longer lasting fat graft. If previously lipotransfers fat effect lasted 9months to 2 years, with Puregraft it could last upto 5years. 

Method :

1. Local anesthesia is given in the form of tumescent to the areas where fat is to be harvested. These are usually the tummy, inner and outer thighs and sometimes upper arms. 

2. Harvesting  of fat is conducted after several minutes and once the local tumescent anesthesia has taken effect.  

3.  A nurse helps to sort out the fat grafts while DrKamakura continues his fat harvesting.

The Puregraft 800 bag is positioned properly ready to receive the extracted fats.

4.  Once the pinkish yellow extracted fats were put into the Puregraft bag, it only takes 10-15 min for the process to complete. DrKamakura's skill in fat extraction is superb. 

5. The purified fat is then transferred back into 60cc syringes , ready to be implanted into face or breasts. In this case it was for breasts. 

6. It is important to know the anatomy of the breasts to ensure the correct level of placement of the puregrafted fat.

7. Local anesthesia is also given to the breasts first. The fat graft is then gently placed into each breasts using special curved instruments. 

The breasts really looked beautiful and natural after the procedure was complete. The best part is it also felt natural!

After a we finished , we went for a good traditional Japanese dinner with the team where certificates and gifts were presented. What a fun and fruitful day it was. I am excited to perform fat grafting using this Puregraft technique back in Malaysia.
